The event planner hopes to meet or coordinate most items in these questions. However, some items will need to be assigned to others. (i.e. graphic design, IT technology support, etc). Please complete this form so event planner can review and contact you for more information, if needed. If there are other conflicting events, you will be promptly contacted with information on alternative routes for assistance. For events that include a registration fee (for example a symposium or course), the recharge rate for the event planner is $24.00/hr.
